Work Experience Qualifications: 1. Submit a copy of current physical exam within last 2 years with current MMR and DT immunizations. 2. Graduation with a degree in dental surgery(DDS) or dental medicine(DMD)from a US or Canadian school approved by the council on Dental education, ADA. 3. Submit a current resume, license and all documentation required for credentialing. 4. Submit at least three letters of reference. Performance Questionnaire 1. Please describe ability to make oral and radiographic exams of the oral cavity. 2. Please describe ability to independently treat patients having dental health problems of greater than usual difficulty. 3. Please describe ability to implement an oral prevention program. 4. Please describe knowledge of Dental infection control procedures such as sterilization procedures and use of personal protective equipment.
Job Description The Aberdeen Area Indian Health Service is currently seeking a Dentist to perform services for two weeks consecutively; a total of three 3 times during this fiscal year. Tour of duty hours will be from 8:00 am to 4:30pm at the Ft. Yates PHS, IHS Hospital in Ft. Yates, North Dakota and other times services will be needed at McLaughlin Health Clinic in McLaughlin, South Dakota. This unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other pertinent to the performance of the purchase order. Location will be at the McLaughlin Clinic in McLaughlin, South Dakota and Fort Yates PHS, IHS Hospital in Fort Yates, North Dakota.
