RN CASE MANAGER needed in for NORTHERN CALIFORNIA LTC FACILITY:Our national healthcare organization operates several facilities throughout northern California and we are seeking a Case Manager for each of our facilities. Our Case Managers will coordinate the management of patient care while following patients throughout the continuum of care. The position will ensure best utilization of resources, service delivery and compliance with all external review agencies. SALARY, BENEFITS & RELOCATION$65,000 - $90,000 annual base salary, depending on experience and facility size, plus bonus. Excellent benefits and relocation assistance is also offered. POSITION REQUIREMENTS:1. Minimum degree of Associates (RN)2. Professional licensure as Registered Nurse.3. Prefer Commission for Case Manager Certification (CCMC) and/or Association of Rehabilitation Nurses (ARN) Certification.4. At least 3 years' clinical experience.5. Prefer experience in Case Management, Quality Management, Utilization Review, or Discharge Planning.POSITION RESPONSIBILITIES:1.
